Follow the steps below to complete the task.

Step 1: Visit the website link and a Google Form will open.

Step 2: You need a Gmail account to proceed, sign in with your email to participate.

When filling the form, pay attention to this key information so as to get credited.

Step 3: Choose "IFO" when you get to this section "Which local government area in Lagos State do you currently reside?" (As proof, take a screenshot of that section)

Step 4: Select N/A for service providers you do not use currently

Step 5: On the last page of the form, click on "SUBMIT" button and take a screenshot of the success page.
